چکیده

PurposeThis study investigated the role of one-stage stereo-array radiofrequency thermocoagulation based on stereotactic electroencephalography in treatment pediatric hypothalamic hamartomas.MethodsWe analyzed clinical data 28 patients with hamartoma. A high-density focal electrode implantation strategy was adopted. Stereotactic guided bipolar coagulations were performed between two contiguous contacts same electrode, or adjacent different electrodes. Patients hamartoma divided into groups size (maximum diameter < 15 mm vs maximum ? mm). Numbers implanted electrodes and used for documented. Seizure outcome evaluated according to Engel’s classification.ResultsSurgical procedures well tolerated no repeated surgery performed. The median number significantly (p = 0.0009), as where applied 0.0006). Moreover, positively correlated volume (Spearman's rho 0.7074, p?0.0001 Spearman's 0.7435, p?0.0001, respectively). overall seizure-free rate 82.1 %, 92.9 % having favorable outcomes at least 12 months follow-up. not statistically significant 0.3138).ConclusionOne-stage using cross-bonded ablation range expansion is a safe effective surgical approach children

Lesion guided radiofrequency thermocoagulation (L-RFTC) for hypothalamic hamartomas, nodular heterotopias and cortical dysplasias: Review and perspective

Lesion guided radiofrequency thermocoagulation (L-RFTC) via stereotactically inserted coagulation probes is a further development of stereotactic thermocoagulation thalamotomy and stereo-EEG guided RFTC. In this method epileptogenic lesions detected via magnetic resonance imaging (MRI) move to the center of coagulation planning. Two surgical strategies can be applied: lesion disconnection and l...

Endoscopic Treatment of Hypothalamic Hamartomas

Hypothalamic hamartoma (HH) is a benign indolent lesion despite the presentation of refractory epilepsy. Behavioral disturbances and endocrine problems are additional critical symptoms that arise along with HHs. Due to its nature of generating epileptiform discharge and spreading to cortical region, various management strategies have been proposed and combined. Surgical approaches with open cra...

Hypothalamic hamartoma causing gelastic seizures treated with stereotactic radiofrequency thermocoagulation.

PURPOSE To present a case of small hypothalamic hamartoma (HH) causing gelastic seizures and treated with stereotactic radiofrequency thermocoagulation. CASE REPORT A 22-year-old man presented with intractable gelastic seizures and focal seizures refractory to medical treatment. Magnetic resonance imaging showed a 6 mm x 6 mm x 7 mm sessile intraventricular HH. Under local anesthesia, four in...

Hypothalamic Hamartomas: Two Cases

This is a MRI study of two patients presented with history of gelastic seizures since many years ago. Plain and post intravenous gadolinium multiplanar MRI imaging of the brain is performed. It showed well-defined non-enhancing mass lesions in the region of hypothalamus and tuber cinereum. It showed signal intensity similar to the gray matter and imaging diagnosis of hypothalamic hamartoma is m...

Neuronavigation-guided Frameless Stereoelectroencephalography (SEEG)

Stereoelectroencephalography (SEEG) is an invasive surgical procedure used to identify epileptogenic zones. The combination of both subdural grids and depth electrodes (DEs) is currently used for invasive intracranial monitoring in many epilepsy centers. To perform DE implantation, some centers use frame-based stereotactic techniques and others use stereotactic robotic techniques. However, not ...
